Surgical Treatment of Hilar and Intrahepatic Cholangiocarcinoma (Updates in Surgery)

Cholangiocarcinoma is the second most frequent primary neoplasm of the liver and its incidence is increasing in Western countries. These neoplasms arise from the biliary tract and can be categorized according to their anatomical location as intrahepatic and extrahepatic cholangiocarcinomas. The prognosis of these neoplasms is poor and most patients are not suitable for curative resection at the time of diagnosis ,due to advanced stage of tumor. Complete surgical resection still remains the only effective and potentially curative therapy. Both intrahepatic and hilar cholangiocarcinoma often requires major liver resection to obtain a radical treatment. In recent years the improvements in non- invasive diagnostic imaging modalities, a better understanding of liver anatomy, the improvements of preoperative optimisation of liver function (preoperative biliary drainage, portal vein embolization), the increased safety of liver surgery have gradually improved the outcome. Moreover, aggressive surgical approach could increase curative surgery. Still now the role of adjunvant therapies and liver transplantation are not well clarified. Saturday, June 4, 1955, my parents were introduced to a beautiful baby girl they named Linda Denise Hunt. This was my best day ever. My parents were Arnett Hunt, thirty-two years old, and Adrienne Hunt, twenty-one years old. I was the first girl born on my father?s side of the family in thirty-five years. I think it was the best day ever for them too! I came from educators and entrepreneurs. In a small southside community of Chicago, I attended Morgan Park High School, and after graduating, I struggled with trying to find my purpose in life, asking myself, am I smart enough to become an educator, or will I have the skills and/or talent to become an entrepreneur? Through adoptions and extended families, my family grew by leaps and bounds. I learned at an early age how to open myself up to people, love others, and pursue a dream of my own. I watched my mother raise the six children left at home after the death of my father some thirty years ago, who died from three ruptured brain aneurysms, which might not have happened had today?s knowledge been available then. This book will take you to a world of the unknown and introduce you to a world some of us heard about but never thought could happen to us. Against Bioethics (Basic Bioethics)

Governments, health professionals, patients, research institutions, and research subjects look to bioethicists for guidance in making important decisions about medical treatment and research. And yet, argues Jonathan Baron in Against Bioethics, applied bioethics lacks the authority of a coherent guiding theory and is based largely on intuitive judgments. Baron proposes an alternative, arguing that bioethics could have a coherent theory based on utilitarianism and decision analysis. Utilitarianism holds that the best option is the one that does the most expected good. Decision analysis provides a way of thinking about the risks and trade-offs of specific options. Like economics, utilitarian decision analysis makes predictions of expected good in complex situations, using data when possible, and focusing human judgment on the issues relevant to consequences. With such a guiding theory, bioethics would never yield decisions that clearly go against the expected good of those involved, as some do now. Baron discusses issues in bioethics that can be illuminated by such analysis, including "enhancements" to nature in the form of genetics, drugs, and mind control; reproduction; death and end-of-life issues, including advance directives, euthanasia, and organ donation; coercion and consent; conflict of interest and the reform of internal review boards; and drug research. Although Baron opposes current practice in bioethics, he argues that by combining utilitarianism and decision analysis, bioethics can achieve its aims of providing authoritative guidance in resolving thorny medical and ethical issues.

The Funding of Young Investigators in the Biological and Biomedical Sciences

This book brings to light trends in the support of life scientists beginning their professional careers. In 1985, 3,040 scientists under the age of 36 applied for individual investigator (R01) grants from the National Institutes of Health, and 1,002 received awards, for a "success rate" of 33%. In 1993, 1,389 scientists under the age of 36 applied for R01 grants and 302 received awards, for a success rate of 21.7%. The markets for electricity, gas and temperature have distinctive features, which provide the focus for countless studies. For instance, electricity and gas prices may soar several magnitudes above their normal levels within a short time due to imbalances in supply and demand, yielding what is known as spikes in the spot prices. The markets are also largely influenced by seasons, since power demand for heating and cooling varies over the year. The incompleteness of the markets, due to nonstorability of electricity and temperature as well as limited storage capacity of gas, makes spot-forward hedging impossible. Moreover, futures contracts are typically settled over a time period rather than at a fixed date. All these aspects of the markets create new challenges when analyzing price dynamics of spot, futures and other derivatives. This book provides a concise and rigorous treatment on the stochastic modeling of energy markets.
